To request a Penalty Reversal on your water/sewer bill, complete the Customer Penalty/ Adjustment Request form. You can return this form to us by email, fax, mail or drop box. The Finance Committee will review your account payment history along with your completed form. Penalties will not be reversed more than once every 3 years. You will be contacted by email with the Finance Committee's decision. (Rev 4 11/03/2016)

This packet contains the required forms for seasonal meter installation in Washington Township for the 2017 season. The completed Memorandum of Understanding can be submitted: in person, dropbox at Township Hall, by mail, or by email. NOTE: Backflow assemblies must be tested every year.

You must file a written claim with the Department of Public Works within 45 days of a storm or sanitary sewer overflow or back-up. Failure to provide the completed form within the specified timeframe will prevent recovery of damages.

Irrigation / 2nd meter permits are issued at the Department of Public Works to the homeowner or their contractor. A Plumbing permit, also required, is issued by the Building Department at the counter to the homeowner or a licensed plumber. A Backflow Certified plumber or irrigation specialist must test the backflow device on your irrigation system. Submit Backlfow Test results to the Department of Public Works prior to scheduling the installation of the irrigation / 2nd meter. (rev. 06/2017)
