  • Irrigation / 2nd meter packet
    Irrigation / 2nd meter permits are issued at the Department of Public Works to the homeowner or their contractor. A Plumbing permit, also required, is issued by the Building Department at the counter to the homeowner or a licensed plumber. A Backflow Certified plumber or irrigation specialist must test the backflow device on your irrigation system. Submit Backlfow Test results to the Department of Public Works prior to scheduling the installation of the irrigation / 2nd meter. (rev. 01/2021)
  • Contractor Registration 2022-2023
    Contractor registration for Sewer and/or Water installation in Washington Township expire on March 31 every year. Details required for insurance and bond as well as the Registration form , are included in this packet.
  • Application for Development and Construction Permit rev 2019
    The Department of Public Works requires this form to be submitted at the time development plans are submitted for engineering review.
  • DPW application for SINGLE FAMILY HOME
    Submit completed form to the Building Department when applying for ANY DPW permits on Residential properties, which includes New Construction, Grade Changes, in-ground pools, etc.
  • As Built Review Checklist rev 11.07.2016
    The Department of Public works requires this completed form to be submitted with As Built Plans for review. The As Built Plans must be submitted and approved within 6 months of underground construction being completed or building permits may not be issued.
  • Certification of Final Grade cover letter rev. 2016
    For Final Grade: Email to the DPW one (1) as-built plot plan and a signed and sealed Certificate of Final Grade from a registered civil engineer or surveyor.
  • As Built Plot Plan  SAMPLE
    This is an As Built Plot Plan sample. as Built Plot Plans are required by the Department of Public Works, along with a Final Grade Certification for Final Grade inspections.
  • Soil Removal License application
    The Superintendent of Public Works oversees mining and soil removal operations in the Township. The Planning Commission approval and Township Board approval may be required for new sites.

    Use this application to request a disconnect from public water and/or public sewer services. The completed application and deposit are to be submitted to the Department of Public Works (DPW).

  • Demolition Abandonment Disconnect Services
    Prior to demolition, the DPW requires an application to disconnect the municipal water and/or municipal sewer.  A deposit is required.

  • Customer Penalty/ Adjustment Request
    To request a Penalty Reversal on your water/sewer bill, complete the Customer Penalty/ Adjustment Request form. The Finance Committee reviews the completed form as well as your account payment history for the past three (3) years. In general, a penalty removal request will be granted not more than once (1) per three (3) year period for customers with no history of late payments. You will be contacted by email with the Finance Committee's decision. (Rev6 Nov2021)
  • Seasonal Sprinklers 2022
    This packet contains the required forms for seasonal meter installation in Washington Township. The completed Memorandum of Understanding can be submitted: in person, dropbox at Township Hall, by mail, or by email. NOTE: Backflow assemblies must be tested every year by an ASSE certified tester.
  • Storm or Sanitary Sewer Back-up Claim Form
    You must file a written claim with the Department of Public Works within 45 days of a storm or sanitary sewer overflow or back-up. Failure to provide the completed form within the specified timeframe will prevent recovery of damages.
